Registration Information
The cost for both UNOE Neighborhood Association members and guests is $18.00 per person and must be paid in advance by the deadline of Thursday, October 10, 2019. View your Dinner Choices.
FORMS
Neighborhood Associations registering as a group should download the Sparkplug Banquet Registration Packet for all the forms needed.
Individuals/Groups/Organizations not registering with a Neighborhood Association can use the Basic Registration Form.
Sparkplug Award Nominations
Each Neighborhood Association may nominate oneindividual, group, business, or organization that resides in your neighborhood to receive your neighborhood Sparkplug Award at the banquet.
Reflect on who has gone above and beyond to help their neighbors and their neighborhood. Consider “Why does this person or group deserve this award?” as you select your Sparkplug Award nominee.
Nominations must be received at the UNOE office by Thu., October 10, 2019. Use the Sparkplug Award Nomination Form in the registration packet to submit your Neighborhood’s award winner. Take a peek back at 2017 winners and 2018 winners.
Door Prizes: Baskets & Wreaths
The basket raffle has proven to be one of the highlights of the evening!
Neighborhood Associations are asked to participate by making a themed basket or wreath for a door prize which those in attendance are eligible to win.
Use the Door Prize Donation Form in the registration packet to let the UNOE office know of your donation by Thu., October 10, 2019.
Neighborhood Photos Sought
Each Neighborhood Association may submit up to 5 photos of persons, projects, buildings, etc. that show something positive about your association’s work and/or your neighborhood’s assets or both, while trying to tie in with the banquet’s theme.
A one-line caption should accompany each photo. The photos with captions will be used in a PowerPoint presentation at the banquet.
Photos must be delivered/mailed to the UNOE office or emailed to office@unoevansville.org by Thursday, October 10, 2019.
Sponsorship Ads
The program booklet still has room for a few sponsorship ads. For details on the sponsorship levels, download the Sponsorship Letter. Sponsorship forms and art for ads are due to the UNOE office by Tuesday, October 1, 2019. NOTE EARLIER DATE THAN OTHER DEADLINES (Dimensions for ads are noted on the form. If you’ve sponsored in the past, please note the changes for this year.)
